Cédric Krier 515051ac21 Remove unique constraint on model and field access 
Removing this constraint makes Tryton more modular.
As sometimes access must be added by module for technical reason, we should not
worry about duplication of such access entry.

Cédric Krier 0c652a7473 Replace (Field, Operator, Operand) by Domain in Rule 
The current implementation requires to query the fields description in English
to allow to convert the operand into value. This is process is quite expensive
on the translation table.

Laurent Defert d06955421c Remove deprecated reports initialization 
Remove code that used to initialize reports with undefined classes.
This initialization is done from Pool.get() method instead.
